When you are scheduled with our office, we will email you the intake paperwork required for our first visit together.

Please print and fill out our intake paperwork to the best of your ability. When you bring in the completed paperwork on your first visit we can use your allotted time in full to begin our work together.

If you do not have access to a printer then please arrive fifteen minutes prior to your scheduled appointment and we will provide you the paperwork.
